How should patients manage bladder problems, what proportion of Parkinson’s patients experience them, and how do lifestyle adjustments compare with medication?

This is a “systems” question, and I am a systems man.

I’m Prakorb Panmanee, but on my YouTube channels and travel blog, I’m “Mr. Hotsia”. My first life was as a systems analyst for the Thai government. I was trained to see the “code” behind the “program”—how one small bug, one single “error” in a line of code, can crash an entire complex system.

My second life, for the last 30 years, has been on the road. I’ve been a “luy-deaw” (solo traveler). I’ve walked, motorbiked, and taken local buses to every single province of Thailand and deep into the local lives of Laos, Cambodia, Vietnam, and Myanmar.

My third life is as a digital entrepreneur and health marketer. My “systems analyst” brain woke up again, and I started researching the data behind the “natural” health products I was promoting—work that eventually won me a ClickBank Platinum award.

In all these lives, I’ve seen the “invisible” crashes.

I’m not talking about the visible “bug” of Parkinson’s (the “hardware” tremor). I’m talking about the “crash” that no one sees. The “crash” that happens in the “system’s” most “private” “hardware.”

The bladder.

🌏 H3 My “30-Year Observation” (The “Local Wisdom” vs. The “Crash”)

As a “luy-deaw” (solo traveler), my entire life is “systems management.”

I have a crystal-clear memory of a 10-hour bus ride in rural Laos. It was not a “VIP” bus. It was a “local” bus. It did not have a “hardware” (a toilet). I knew the “system.”

What did I see? The “local wisdom.” An hour before the bus, the “local” 80-year-old “Yai” (grandmother) stopped drinking. She “ran her maintenance program” (she used the toilet) right before she got on. She knew the “program” (the “system”) of the bus. She managed her own “system” to match it.

This is “local wisdom.” It’s proactive systems management.

Now… contrast this with the Parkinson’s “crash.”

The “bug” (Parkinson’s) corrupts the “code.” The “hardware” (your bladder) might be fine. But the “signal” (the “wiring”) from the “hardware” (bladder) to the “software” (your brain) is “buggy.”

The “Yai” on the bus “ran” her own “program” (she chose when to go). The PD patient has their “system” “crashed” by a “buggy program” they cannot control.

This isn’t a “hardware” crash. This is a “dignity crash.” It’s the “crash” that stops you from taking the bus ride. It’s the “bug” that steals your “autonomy.” It “shrinks” your “system” (your life) until your “operating system” is just the “four walls of your house.”

This is the “crash” we must “fix.”

📈 H3 The “System ‘Bug’ Report” (The Data)

As a health marketer, I live on “data.” I “ran the data” on this “bug.”

What proportion of PD patients experience this?

The “data” from my “Western” health research (my “lab”) is shocking. This is not a “rare” “bug.” This is a feature of the “crash.”

The “proportion” is massive. The “data” shows that 30% to 80% (depending on the “bug” you’re measuring) of all PD patients have this “system failure” [S1, S2].

The “bug” has “error codes” (symptoms):

  • “Nocturia” (The “Night-Mode Crash”): This is the most common “bug.” The “system” “runs” the “wake-up ‘program'” 3, 4, 5 times a “system cycle” (night). This “crashes” your other “hardware” (your sleep “system”).
  • “Urgency” (The “Panic ‘Bug'”): This is the “bug” that “crashes” your “dignity.” It’s the “ghost signal” that says “CRASH IMMINENT” (you must go now).
  • “Frequency” (The “Lagging ‘Program'”): The “system” “runs” the “go” “program” 10, 15 times a day.

This “data” proves this is not a “user” (patient) “error.” This is a “system-wide hardware/software failure” caused by the “root bug” (PD).

🧠 H3 The “Buggy ‘Code'” (Why This “System” Crashes)

As a systems analyst, this is the “code.”

This is not a “plumbing” (“hardware”) “bug.” Your “bladder ‘hardware'” is (usually) fine. This is a “neurological ‘wiring'” “bug.”

  1. “The ‘Code'” (Dopamine): The “bug” (PD) “corrupts” the “dopamine ‘code'” in your “software” (your brain).
  2. “The ‘Signal'” (The “Wire”): “Dopamine” is the “signal” that controls the other “signal” (Acetylcholine) that “runs” your “hardware” (your bladder).
  3. “The ‘Crash'”: When the “Dopamine ‘code'” is “buggy,” the “Acetylcholine ‘signal'” “crashes.” It goes haywire. It “panics.” It “runs” the “SPASM ‘PROGRAM'” when it’s not supposed to.
  4. The “Error Message”: That “spasm ‘program'” is the “Urgency ‘Bug'” you feel.

The “bug” is in the “software” (the brain), “corrupting” the “wiring” (the nerves), which “crashes” the “hardware” (the bladder) [S3].

This is why the “fix” is so complex.

⚖️ H3 The “Showdown”: “Patch” vs. “Process” (Meds vs. Lifestyle)

This is the core of the review. As a systems analyst, I must compare the “fixes.”

  • The “Western Fix” (Medication / The “Patch”): This is the “Chemical Patch.” The “System Admin” (your doctor) “installs” a “patch” (a pill) to “fix” the “buggy signal.”
    • “The Patch” (e.g., Anticholinergics / Oxybutynin): This “patch” blocks the “haywire ‘signal'” (Acetylcholine). It mutes the “alarm” (the spasm) [S4].
    • The “Problem”: As an analyst, I see this is a “dirty ‘patch’.”
    • It blocks the “signal” everywhere. It “crashes” your other “hardware.”
    • It causes “new bugs”: Dry Mouth, Constipation… and the “Fatal Error”: “Brain Fog” (Cognitive Impairment) [S5].
    • This is a terrible “trade-off.” You cannot “install” a “patch” that “fixes” the “bladder ‘bug'” by worsening the “brain ‘bug'” (which PD already causes).
  • The “Local Wisdom” (Lifestyle / The “Process”): This is the “System Update.” This is the “Hotsia” method. This is the “Lao ‘Yai’ on the bus” logic. Instead of “patching” the “buggy ‘hardware’,” you “re-write the ‘code'” (your behavior) around the “bug.” This is the “proactive” fix. It is “clean code.” It has zero “side effects.”

As an entrepreneur (I built sabuy.com and Kaprao Sa-Jai ), I always choose the “clean ‘code'” with the highest “Return on Investment” (ROI). The “ROI” of the “patch” (meds) is low (it fixes one “bug,” causes another). The “ROI” of the “process” (lifestyle) is high (it fixes the “crash” and gives you back dignity).

📊 H3 My “Systems Analysis” Showdown (Table 1)

As an analyst, I live for tables. This is my “bug report” on the “fixes.”

“System” Strategy The “Bug” It Fixes (My Analysis) The “Side Effect” (The “Hidden Crash”) My “Hotsia” Take (The “ROI”)
“The Patch” (Meds) (e.g., Anticholinergics) “The ‘Alarm'” (The “spasm ‘bug'”). (A reactive “mute button”) [S4]. “FATAL ERROR.” (Crashes “software”—Brain Fog, Dry Mouth, Constipation) [S5]. Low ROI. This is “dirty ‘code’.” It’s a bad trade.
“The Process” (Lifestyle) “The ‘System’ ‘Crash’.” (The behavior). (A proactive “system update”). Zero. (“Clean code”). High ROI. This “fix” only “patches” the “problem” and restores “dignity.” This is “smart code.”
“The ‘Hotsia’ Way” (“Full System”) “Process” First. “Patch” Second. (None, if the “Process” works). Highest ROI. You “run” the “clean ‘code'” (Lifestyle) first. You only use the “dirty ‘patch'” (Meds) if the “Process” fails.

💡 H3 The “Hotsia Way”: A “Full System Overhaul” (How to Manage)

This is the “how-to” (Q1). This is the “clean code” you can “run” today. This is the “Hotsia” “systems manual” for “de-bugging” the “crash.”

  • 1. “Install” the “Local Wisdom” (Timed Voiding): This is my “Lao bus” hack. This is Priority #1. The “bug” (PD) wants to “run the ‘program’.” You cannot let it. You must “run the ‘program'” first. The “Code”: “Run” the “maintenance ‘program'” (go to the toilet) on a schedule (e.g., every 2 hours). Do not wait for the “buggy ‘alarm'” (the “urgency”). You “re-write” the “code” with a schedule. You are “smarter” than the “bug.”
  • 2. “De-Bug” the “Inputs” (The “Kaprao Sa-Jai” Fix): My “Kaprao” philosophy is “clean inputs.” A “buggy ‘system'” (PD) cannot “run” on “buggy ‘inputs’.”
    • “Corrupted ‘Input'” #1: Caffeine. (Coffee, Tea, Soda). This “input” is an “irritation ‘bug’.” It “pings” your “hardware” (bladder) and causes the “spasm ‘program’.”
    • “Corrupted ‘Input'” #2: Alcohol/Sugar. (Also “irritation ‘bugs'”).
    • The “Hotsia” Fix: “De-bug” your “inputs.” “Run” a “test.” Stop “inputting” all “caffeine ‘code'” for three days. See if your “system” “crashes” less. (This is the “health marketer” “Blue Heron” fix—find the “buggy input”!)
  • 3. “Re-Code” the “Night-Mode ‘Crash'” (Nocturia): This is “systems logic.” If the “system” “crashes” at “night,” stop “inputting” “data” (fluids) before “shutdown.”
    • The “Code”: Stop all “inputs” (drinks) 2-3 hours before your “system shutdown” (sleep).
    • This is the easiest “patch” in the world.
  • 4. “Patch” the “Hardware” (Pelvic Floor): The “bug” is in the “wiring,” but you can strengthen the “hardware.”
    • The “Patch”: Pelvic Floor Exercises (Kegels).
    • The “Code”:Two ‘Programs'” (Table 2): This is my “Kaprao Sa-Jai” “clean input” “menu.” This is the “local wisdom” I’ve seen for 30 years vs. the “modern ‘buggy’ inputs.”
“Buggy Input” (My “Field” Obs.) The “System Error” It Causes (My “Analysis”) The “Hotsia” Fix (“Clean Code”) The “Local Wisdom” (Why it Works)
Caffeine (Coffee / Strong Tea / Soda) “The ‘Irritation’ Bug.” It “pings” the “hardware” (bladder) and causes the “Urgency ‘Program'” to “run.” “De-bug” it. “Run” the “program” without this “input.” (Switch to herbal tea). My “Lao ‘Yai'” (grandmother) on the bus was drinking weak tea. She knew “strong” “inputs” were “buggy code.”
Alcohol (Especially Beer) “The ‘Double’ Crash.” It’s both an “Irritation ‘Bug'” and a “Diuretic ‘Bug'” (makes more “waste”). “Delete” this “code.” (Especially at night). This is “local wisdom” 101. No “system” (PD or healthy) “runs” well on this “corrupted” “input.”
Too Much “Data” (Fluid) at Night “The ‘Night-Mode Crash'” (Nocturia). You are overloading the “system” while it’s “shutting down.” “Time” your “inputs.” “Stop ‘data’ (fluid) ‘input'” 2-3 hours before “shutdown” (sleep). This is the “Lao bus” “code.” Manage the “input” before the “event” (the “night”).
“System” Constipation “The ‘Hardware’ ‘Crash’.” “Full ‘hardware'” (bowel) presses on “other ‘hardware'” (bladder). “Fix” the “Constipation ‘Bug’.” (More fiber, more daytime water). This is a “systems” “crash.” One “crashed” “sub-routine” (bowel) will “crash” the “sub-routine” (bladder) next to it.

🙏 H3 My Final Word: “Patch” Your “Dignity” First

I am a “luy-deaw” (solo traveler). My “master program” is independence. Autonomy. I must be in “control” of my “system.”

The “bug” (Parkinson’s) attacks your “hardware” (body). The fear of the “crash” (the “accident”) attacks your “software” (your freedom).

That “fear” “crashes” your “system” worse than the “bug” itself. It “shrinks” your “world” (stops you from traveling).

This is why the “Process” (Lifestyle) is the smarter “system” than the “Patch” (Meds). The “Process” (the “Lao bus” hack) gives you back “control”. The “Patch” (the pill) just “mutes the ‘alarm'” while causing new “bugs” (brain fog) [S5].

Be the “Systems Analyst” of your own “system.” “Install” the “local wisdom.” “De-bug” your “inputs.” “Re-write” your “code.”

You “run” the “program.” Don’t let the “bug” run you.

❓ H3 (Your) Frequently Asked Questions

H3: You’re not a doctor. Is this bladder ‘bug’ really Parkinson’s, or just “old age”? (My Answer: As a systems analyst, it’s the bug. “Old age” “hardware” (a healthy bladder) works. The “bug” (PD) specifically “crashes” the “dopamine ‘code'” that “runs” the “wiring” to the “hardware” (bladder) [S2, S3]. It’s a “PD ‘system error’,” not an “age ‘hardware’ error.”)

H3: Are the “patches” (medications) bad? (My Answer: Not “bad.” They are “dirty ‘code’.” They are very “buggy.” They “patch” one “bug” (the spasm) but cause a “new, worse bug” (the “brain fog”) [S5]. As a systems analyst, this is a terrible “trade-off” for a “system” (PD) that already has a “brain ‘bug’.” The “Process” (Lifestyle) is “clean ‘code.'”)

H3: What is “Nocturia”? It’s my worst “bug.” (My Answer: It’s the “Night-Mode Crash.” It’s the most common “bug” in the PD “system” [S1, S2]. The “fix” is the “Hotsia” “systems logic”: Stop “inputting ‘data'” (fluids) 2-3 hours before “system shutdown” (sleep).)

H3: You traveled 30 years. What’s the “Hotsia” travel “hack” for this? (My Answer: My “Lao bus” hack: “Timed Voiding.” And “System Redundancy.” As an analyst, I know a “system” can “crash.” So I have a “backup ‘patch’.” Be “proactive.” Wear the “backup ‘patch'” (an adult brief / pad). This “patches” your software (your fear). It gives you the “dignity” to take the 10-hour bus ride. Control the “system.” Don’t let the “bug” control you.)

H3: What’s the one “fix” I can “run” today? (My Answer: The “Kaprao” fix. “De-bug” your “inputs.” Stop drinking coffee and soda for 3 days. That’s “Step 1” of “running a ‘diagnostic’.” See what your “system” “reports” back. It’s “clean code.” It’s free.)
